In-Home Family Support Services


Services

Residential-based habilitation serves designed to enable the person to remain in the family home and are directed towards increasing and maintaining the person's physical, intellectual, emotional, and social functioning.


Eligibility

To be eligible for In-Home Family Support Services an individual must:
  • Have a documented diagnosis of mental retardation or a related condition.
  • Live in an Intermediate Care Facility for Persons with Mental Retardation (ICF/MR) or be assessed, through a county screening process, to need the level of services provided in an ICF/MR.
  • Make an informed choice requested MR/RC waiver services instead of ICF/MR services.
  • Be eligible for Medical Assistance (MA) based only on his or her income and assets, even though the applicant may live with spouse or parents.
  • Need a 24-hour plan of care and an individual service plan assuring health and safety.

How to Apply

Contact the county social services agency where the person with mental retardation or related conditions resides and ask about semi-independant living services. The county can assist applicants in the application process. County Web Sites
